All New Patients must have a New Patient Registration form filled out and on file with us at Crosswinds Animal Hospital. By signing and submitting this form you agree to pay any fees and charges associated with exams and treatments for your pet. Also, all clients who fail to show will be considered a No Show and will be charged a $50 No Show Fee. All New Clients will be charged a $50 fee prior to scheduling an appointment with a doctor. This fee will be applied to the account and will go towards your first appointment. In the event a new client fails to show, the prepaid fee is non-refundable and you will be required to prepay an office visit again prior to rescheduling.

No Show and Cancellation Policy
As of January 15th 2022 an update to our No Show Policy will take effect for all new and existing clients.
This policy is being put in place to ensure all of our patients are able to receive the highest level of care we can offer.
We ask that you call 24 hours in advance to cancel your appointment if you know you will be unable to attend. By providing notice of cancellation we can accommodate another patient with your appointment slot. When you do not show to your appointment, that creates an unused appointment time another patient may have benefited from.
New Clients:

New Clients will be charged a $50 scheduling fee to be paid over the phone during the scheduling of your appointment with one of our Doctors. This fee will be applied to your account and credited towards your first visit.

If a new client fails to show, the pre-paid fee will be non-refundable and you will be required to pre-pay the scheduling fee again if you choose to reschedule your appointment.
Existing Clients:

Existing Clients who fail to show will be considered a "No Show" and a fee of $50 will be applied to your account. This fee will need to be paid prior to scheduling your next appointment.

If, for any reason, you need to cancel please contact our office 24 hours in advance to avoid a "No Show" fee being added to your account.

Existing Clients who fail to show to (2) appointments will be required to prepay a $50 fee prior to scheduling any future appointments. This prepaid fee will be applied to your account and credited towards your scheduled appointment. If you fail to show to that appointment the $50 scheduling fee will need to be paid again prior to scheduling another appointment.

Any Client failing to show for a scheduled surgery will have a fee of $100 applied to your account. This fee will need to be paid prior to rescheduling your pets surgery.

Crosswinds Animal Hospital reserves the right to refuse service and terminate the doctor/client/patient relationship at any time for non-compliance relating to both patient care and failing to show to appointments.
